Your aging life care expert serves as a guide and advocate to assist clients in attaining maximum functional potential in a suitable environment. Your CCM also coordinates care in a cost efficient manner, utilizing local resources and support systems based on the client’s needs and goals. Free from conflicts of interest, CCMs abide by a strict code of ethics and nationally reviewed standards of practice.
